Subject matter expert should be the facilitator and his/ her qualifications need to be submitted. Facilitator/ assessor qualifications need to be submitted with certified certificates. Moderator qualifications need to be submitted with certified certificates.
Assessment tool for the assessment of learners need to be submitted with learner guide.
Moderator guide to be submitted with assessment tool. 2 x LEEASA Subject matter experts are to attend the course or activity to assess the course or activity using the annexure A and annexure B for the vetting and the verification of a CPD course or activity.
